Ejection/Incident Report

Important Information to remember

The purpose of this incident/ejection form is to provide a simplified way for umpires to report incidents and ejections that occur during any game the NFUA is providing umpire services. The NFUA cannot correct problems on the field if they are not reported. Please try to have your report sent to the office as soon as you can after your assignment is complete and no later than 24 hours.It is recomended that you have a hard copy or a notebook to make notes when the incident/ejection took place.
